楊康之子楊過(陳曉飾)自幼流落江湖，被故人郭靖(鄭國霖飾)及其妻黃蓉(楊明娜飾)收留後送到了全真教磨練。反叛的楊過忍受不了折磨，逃出全真教而誤入古墓派，繼而被小龍女(陳妍希飾)收留，傳授武功。後因李莫愁(張馨予飾)攻入古墓而面臨生死，二人由師徒之誼發展成刻骨銘心的愛情。
蒙古鐵騎即將南下，郭靖等人等難以匹敵，關鍵時刻，小龍女和楊過無意中捲入紛爭，並打敗了金輪法王(黑子飾)。但好事多磨，楊過與小龍女多次聚散，在經歷了武林大會、襄陽鏖兵，絕情幽谷等等險境後，更有十六年的生離死別。痴情的楊過終於在絕情谷底找到了大難不死的小龍女，二人重返世間。此時蒙古皇帝正調集人馬，猛攻襄陽，楊過擊敗了金輪法王，並以飛石擊斃了蒙古皇帝蒙哥，為襄陽保衛戰的勝利立下首功，戰事結束，楊過小龍女告別郭靖等人，帶着神雕，悠然遠去。
